The mirror one I was pretty sure I fixed in the DAX patch. Did you download it?
Not sure about the first, since that doesn't happen for me and it's never been reported before. (Hmmm...)
I also fixed the no exit bug for the random dungeon that never appeared in a patch yet. You can get it here (just place in the PRG folder).
